After enjoying your favorite ButterBurger, crinkle-cut fries, or a delicious frozen custard, you can head over to culver's and share your feedback by entering the 18-digit survey code found on your receipt. It’s important to note that this code should be used soon after your visit as it ties directly to your specific transaction, store location, and visit time.

How It Works

1. Receipt Code
   Each Culver’s  receipt includes a 15-character survey code. You’ll enter this code when starting the survey tellculvers.com

2. Answer Brief Questions
   You’ll be asked about food quality, service, cleanliness, menu selection, and overall satisfaction.

3. Get Validation Code
   Upon completion, you receive a validation code to use with the in-store coupon shown on your receipt typically redeemable for a free frozen custard dish or cone
